How Breakthrough Technologies are Transforming Heart Health: A Journey into Revolutionary Innovations

In a dimly lit cardiac catheterization lab, an interventional cardiologist concentrates intently on a live X-ray of a beating heart.  The doctor carefully guides a long, thin wire, threading it through a small hole near the patient’s wrist towards the heart.  This precise procedure is well-practiced for this physician, who has performed countless heart stent procedures using minimally invasive techniques to open blocked arteries and restore blood flow. 

But this isn’t just any stenting procedure.  The patient on the table has multiple health issues, including severe blockages and concrete-like hardening in two of three main vessels in the heart, severely limiting blood flow.  Additional medical conditions like diabetes, kidney failure, and advanced age make the patient unsuitable for open-heart bypass surgery.  Over the past decade, patients like this have increasingly become the norm in hospitals, and the options to help ​​patients like this have been limited or not optimal. 

Today, interventional cardiologists tackle severe heart disease head-on through several innovations designed to open complex blockages in the heart and legs.  One of the most recent medical breakthroughs, intravascular lithotripsy (IVL), is revolutionizing how heart doctors open hardened arteries lined with calcium.  IVL resembles the original angioplasty balloon but emits pressurized sound waves within the balloon to shatter hard calcium blockages, transforming what could have been a poor quality of life—or even death—into a future with renewed possibilities. 

Interventional cardiologists specializing in high-risk heart procedures routinely see patients who, until recently, would have been told that they needed open heart surgery, which comes with significant risk and a long recovery.  Sometimes, the only option was comfort care with medication to minimize symptoms.  Thanks to continuous advancements in heart disease treatment, there are almost always better options now. 

The story of interventional cardiology is one of relentless innovation.  The field was born nearly 50 years ago with a small heart device made in the kitchen of a Swiss physician, pioneer Dr. Andreas Gruentzig. His breakthrough angioplasty technique, using a tiny balloon to reopen blocked arteries without major surgery, set the stage for revolution.  In the following decades, steady advancements made the original balloon angioplasty procedure far more effective, versatile, and capable of treating complex cardiovascular diseases.  Tiny drills were introduced to shave calcium from artery walls. Metal mesh stents were designed to prop vessels open and later improved with drug coatings to keep them open.  Mini ultrasound cameras provide detailed real-time views inside the heart, and small temporary pumps help failing hearts circulate blood.  These devices, often created through collaborations between physicians, large medical device companies, and small startups, have evolved the field to improve and extend the lives of over a million patients with cardiovascular disease each year in the U.S. alone. 

In this innovative field, IVL represents a significant leap forward for coronary and peripheral arterial disease.  Calcium is an increasingly common and complex component of heart or leg blockages, creating a significant barrier to opening and treating a blocked artery with a stent.  It is estimated that over 40% of patients with coronary disease and nearly all patients with severe blockages below the knee have severe calcification.  This complicates the doctor’s ability to either access the blockage for treatment at all or, if a stent can be placed, it may not be fully opened, which can limit the stent’s performance to keep the vessel open.  

IVL changes that. It shatters even the hardest calcified plaque into tiny fragments within the vessel walls, preventing calcium from breaking off and sending it downstream, potentially creating a new blockage.  IVL makes the vessels more flexible and treatable with standard angioplasty and stenting procedures. In simple terms, IVL is a simple, safe, and effective technology that optimizes stent procedures for the best long-term results. 

What is most exciting about IVL is its ongoing evolution.  In 2016, Shockwave Medical introduced the first and only FDA-approved IVL technology.  Early glimpses of next-generation advancements by a medical device startup company called FastWave Medical promise to advance IVL technology so that even more patients can benefit from this breakthrough technology.  FastWave's IVL devices aim to build on this foundation, offering robust yet low-profile balloons, a new bar for the greatest amount and fastest pressurized sound waves, the ability to treat long blockages, and the capability to tackle the hardest-to-cross blockages. 

No matter where IVL goes in the future, it is already redefining outcomes for patients with cardiovascular disease.  Interventional cardiologists no longer must settle for mediocre results or say there’s nothing more to be done to open a blocked vessel.  There are options.  You, as patients, have options. 

As interventional cardiologists, we celebrate innovations, especially those for the heart and legs, that enable us to lead the battle against cardiovascular disease.  We can confidently say more frequently now, “There’s something we can do for you.”
